To help accommodate all patients, FHL Physical Therapy offers both a self-pay model and an insurance-pay model.
Self-pay & Out-of-Network Insurance
With this option, we will not bill an insurance company, and the client is responsible for full amount per service as advertised on Appointments & Pricing page.
If client is insured but out of network, there may be a possibility of client being reimbursed by their insurance company; we can provide what is called a superbill (detailed, itemized receipt) that client submits to their insurance company to request reimbursement for out of pocket costs. Call the number on your insurance card to determine your out of network benefits.
In-Network Insurance
With this option, we will bill the client’s insurance provider. The client is responsible to pay up front costs based on the terms of benefits on their insurance policy. This will typically involve paying a co-pay, a percentage co-insurance, or a meeting deductible. Your therapist will go over this with you when verifying insurance coverage.
Insurances Accepted & Patient Responsibility
We are currently in network with Aetna plans, and pending a contract with Medicare, Humana, United Healthcare, Blue Cross Blue Shield/Anthem, and Cigna.
If client has to pay a co-pay, co-insurance, or meet their deductible, client is responsible to pay this amount in full to FHL Physical Therapy at time of service.
Payment
Self-pay — payment can be made and 50% down-payment is required to secure appointment. Book through the Appointments and Pricing page. Payment options include Square, Apple Pay, Venmo, CashApp, and Zelle
Insurance — Contact us for verification of benefits. Once verified, FHL Physical Therapy will send a personal billing invoice prior to each session with patient responsibility due
